Health Insurance Cards END in December 2024!

The govenment decided to abolish "Health Insurance Cards" and promote that all citizens to get "My Number Cards". It's an INDIVISUAL NUMBER CARD as health insurance card which is issued by the city office where you live.

If you are a Nirasaki citizen, this is a good news!! You can apply for the card in the hospital while you are there. You can save your time to go to the city hall.

Site: The waiting hall at dermatology in the first floor.

*PLEASE GO TO THE CITY HALL WHEN YOU RECEIVE THE CARD.

【マイナンバーカードの申請＠当院】毎週金曜日9:30-11:30

「健康保険証」が2024年12月に廃止になります!

政府は「保険証」を廃止することを決定し、マイナンバーカードの取得を国民に推進しています。これは、保険証としても使用される個人識別番号カードで、お住まいの市役所で発行されるものです。韮崎市民には朗報！カードの申請が、当院で申請できます！市役所に出向く時間が省けます。

【PEDIATRICS】

We do NOT have a FULL-TIME PEDIATRICIAN from April 2024, but continuously receive infants and childen till the age of 15 by the doctors from Yamanashi University hospital every week days.

For the above reason, patients of pediatrics CAN RECEIVE TREATMENTS WITHIN DAY-TIME CONSULTATION HOURS ONLY but not out of the time or in imergency so that we can not accept patients who need to be hospitalized.

【小児科】2024年4月より正職の小児科医が不在のため、現在は、山梨大学から来ていただいている小児科医が乳幼児から15歳までの患者様を引き続き受け入れております。上記理由により、小児科の患者様は、時間外や救急ではなく、日中の診療時間のみ治療をうけることができます。そのため、入院が必要な患者も対応もできかねます。

The KOYO HOSPITAL and SHIOKAWA HOSPITAL in Hokuto city are also the secondary care hospitals in Kyohoku area.

Surgical Mask Required

All visitors are required to wear surgical masks (unwoven cloth masks) when you visit us. It has proved that is more effective at stopping droplets than the cloth or polyurethane mask.

Temperature check at entrance

Before entering the building, you are required to check your body temperature to find suspected symptoms of covid-19.

入口での体温チェック：新型コロナウイルス感染症の疑い症状を発見するため、建物に入る前に、入口にて体温チェックを行っております。

Visiting Control for Inpatients

You are able to access to the ward with the following conditions.

15 minutes per time / once a week / two persons (maximum)

入院患者の面会制限：下記の条件で病棟への出入りが可能です。

1回15分/一週間に一度/2名(最大)

Vaccine Passport Issuance (vaccination certificate)

This certificate can be used when proof of vaccination is required when you enter foreign countries. If you need an English or Japanese vaccine passport, please visit to the Nirasaki-shi Hoken Fukushi Center (Phone: 0551 23 4310) with the below items. The center is located at the next building of the hospital. FREE CHARGE. Date: 2022.12.13

*A vaccine passport is issued on the day when you applied there.

ワクチンパスポートの発行

これは、諸外国への入国時に求められた場合、ワクチン接種の事実を示す場合に利用するものです。英語または日本語のワクチンパスポートが必要な方は、下記のものをお持ちになり、韮崎市保健福祉センターへお越しください。センターは当院の隣の建物です。無料。

*ワクチンパスポートは申請した当日に発行されます。

お持ちいただくもの：

1. コロナワクチン接種済証、2. 有効期限内のパスポート、3. 在留カード

*Issuance by App 　アプリによる発行

A vaccine passport can be issued by your mobile phone. Please check the website of the digital agency about the details. You need to install the QR cord for App store or Google play which are posted on　middle page of the site. *(1) Your My Number Card, (2) The pin numbers, (3) Your passport are needed.

ワクチンパスポートは、携帯電話からでも発行できます。詳細はデジタル庁のサイトをご覧ください。ページの真ん中あたりにあるMac用またはWindows用のQRコードをインストールする必要があります。発行には、(1)マイナンバーカード (2) カードの暗証番号 (3) パスポートが必要です。
