*Urology, dermatology, pediatrics, cardiology, pulmonology and allergology are by 11:00.

*泌尿器科、皮膚科、小児科、循環器内科、呼吸器内科、アレルギー科は11:00まで。

*If it's over 3 months since you last visited us, you need to go to the reception counter. The check-in machine is not accepted in this case.

休診日：毎週土日祝日、年末年始（12/29~1/3)症状によっては、救急患者を受け入れ。

Surgical Mask Required

All visitors are required to wear surgical masks (unwoven cloth masks) when you visit us. It has proved that is more effective at stopping droplets than the cloth or polyurethane mask.

ご来院の際は、全ての訪問者に対しサージカルマスク（不織布マスク）の着用をお願いしております。布やポリウレタンマスクよりも、より効果的に飛沫を防止することが証明されています。

Temperature check at entrance

Before entering the building, you are required to check your body temperature to find suspected symptoms of covid-19.

入口での体温チェック：新型コロナウイルス感染症の疑い症状を発見するため、建物に入る前に、入口にて体温チェックを行っております。

Visiting Control for Inpatients

You are able to access to the ward with the following conditions.

15 minutes per time / once a week / two persons (maximum)

入院患者の面会制限：下記の条件で病棟への出入りが可能です。

1回15分/一週間に一度/2名(最大)

Vaccine Passport Issuance (vaccination certificate)

This certificate can be used when proof of vaccination is required when you enter foreign countries. If you need an English or Japanese vaccine passport, please visit to the Nirasaki-shi Hoken Fukushi Center (Phone: 0551 23 4310) with the below items. The center is located at the next building of the hospital. FREE CHARGE. Date: 2022.12.13

*A vaccine passport is issued on the day when you applied there.

WHAT YOU BRING:

1. The COVID-19 Vaccination Record

2. Passport within the expiration period.　

3. Resident card

ワクチンパスポートの発行

これは、諸外国への入国時に求められた場合、ワクチン接種の事実を示す場合に利用するものです。英語または日本語のワクチンパスポートが必要な方は、下記のものをお持ちになり、韮崎市保健福祉センターへお越しください。センターは当院の隣の建物です。無料。

*ワクチンパスポートは申請した当日に発行されます。

お持ちいただくもの：

1. コロナワクチン接種済証、2. 有効期限内のパスポート、3. 在留カード

*Issuance by App 　アプリによる発行

A vaccine passport can be issued by your mobile phone. Please check the website of the digital agency about the details. You need to install the QR cord for App store or Google play which are posted on　middle page of the site. *(1) Your My Number Card, (2) The pin numbers, (3) Your passport are needed.

ワクチンパスポートは、携帯電話からでも発行できます。詳細はデジタル庁のサイトをご覧ください。ページの真ん中あたりにあるMac用またはWindows用のQRコードをインストールする必要があります。発行には、(1)マイナンバーカード (2) カードの暗証番号 (3) パスポートが必要です。
